29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   (a) Roof of Harold Johnson near the back of the building: On or about April 21, 2023, the employer exposed employees to struck-by hazards in that eye protection was not used while installing new shingles with nail guns.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Roof of Harold Johnson near the back of the building: On or about April 21, 2023, the employer exposed employees to a fall hazard, in that fall protection was not used on a steep roof while installing new shingles approximately 16 feet 7 inches above asphalt.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Roof of Harold Johnson near the back of the building: On or about April 21, 2023, the employer exposed employees to fall hazards, in that a ladder did not extend 3 feet over the landing used to access the roof.
