29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: WORKSHOP: On/or about April 24, 2023, the employer did not provide the pallet table with a pivot and linch pin to release and insert the damper/shock absorber and the inner spring was not equipped with a lock to lock the inner spring in place to secure the spring to the Spring-Actuated Pallet Carousel Skid Positioner/pallet table when the employee was stacking thin stone onto a pallet resulting in leg injuries to the employee.
