Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  On or about April 24, 2023, at the establishment located at 7559 St Hwy 57, Sturgeon Bay, Wisconsin: the employer failed to protect its employees from unsafe practices in that employees were allowed to use fork extensions attachments on the Bobcat V519 Versa handler forklift (serial number 0005134) without written approval from the forklift manufacturer (or qualified Registered Professional Engineer) for the attachment to be used with the forklift.

If the above equipment is not available at the pier, the employer shall furnish it during the time that he is working the barge.  On or about April 24,  2023, at worksite located at 4335 Park Drive, Sturgeon Bay, Wisconsin: employees were working from a barge in Sawyer Harbor, Sturgeon Bay WI that was not equipped with a life ring and rope of at least 90 feet in length.

On or about April 24, 2023, at the establishment located at 7559 St Hwy 57, Sturgeon Bay, Wisconsin: employer did not have documentation that monthly, annual, and four year inspections of the crane barge were conducted that met OSHA requirements.
