In the Mobile lift, at a temporary job site located at 2701 Hodges Blvd, Jacksonville, FL 32224 (Providence School): On or about April 24, 2023, the employer exposed an employee to head injuries in that a hard hat was not utilized by the employee while cutting tree limbs from the lift. b. On the ground, at a temporary job site located at 2701 Hodges Blvd, Jacksonville, FL 32224 (Providence School): On or about April 24, 2023, the employer exposed two employees to head injuries in that hard hats were not utilized by employees working underneath the employee in the lift that was dropping limbs to the ground for them to gather.
