29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use: a. At a temporary worksite located at 2701 Hodges Blvd, Jacksonville, FL 32221 (Providence School); On April 24, 2023, during construction activities, a Terex Terrain lift , with fork attachments was being used to support a man basket which was not chained or had pins attaching the basket to the mast, exposing employees to a 14 foot fall hazard. The manufacturer prohibits this use.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: a. On the roof of a temporary commercial worksite, located at 2701 Hodges Blvd, Jacksonville, FL 32224 (Providence School): On or about April 24, 2023, the employer exposed its employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using screw guns to install metal roof panels.
