OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, including severe burns, to employees in that the facility, processes, and equipment, including the combustible dust collection system, were not designed, constructed, equipped, and located, to protect employees from fire and explosion hazards: (a) Building 2, Wood Laminator Line - On or about April 25, 2023, the dust collection system, which collected combustible dust from the Wood Laminator Line, did not have fire and explosion prevention/protection systems, including: ? The Donaldson Torit dry-type dust collector, model 64P JD10, serial number IG560601-01, was located inside the building; ? The Donaldson Torit dry-type dust collector, model 64P JD10, serial number IG560601-01 had no deflagration venting or deflagration suppression or inerting systems; ? The dust collection system had no isolation device to prevent fires and explosions/deflagrations from propagating from the dust collector back through the inlet ductwork to Wood Laminator Line machinery and equipment; ? There was no isolation valve between the plastic tote (used to collect combustible dust discharged from the Donaldson Torit dry-type dust collector, model 64P JD10, serial number IG560601-01) and the dust collector. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
