29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about 4/25/2023, in the EDM area; employees were exposed to caught between hazards when operating the Mitsubishi FA 20-S Advance (EDM-03) without the guard.  b) On or about 4/25/2023, in the production area; when employees operate the WF Wells Band Saw (TR-09 horizontal band saw) to cut metal products, employees were exposed to amputation hazards when the unused portion of the blade was unguarded.    c) On or about 5/2/2023, in the CNC area; where the safety interlocked door for the  Fadal 4020 FX ML-07 CNC Machine was broken, allowing the machine to operate with the safety interlocked door "open" exposing employees to caught between and amputation hazards.  ABATEMENT DOCUMENTATION REQUIRED.
