The employer was failing to provide powered industrial truck training to employee(s) on April 21, 2023, at 10699 12th Avenue, Plattsmouth, Nebraska. The employer failed to ensure that forklift operators received forklift training and evaluation as specified in this paragraph (l) before being allowed to operate forklifts independently.
