On or about April 22, 2023, an employee was performing maintenance on the conveyor's chain and sprocket drive. The employee did not follow the developed and documented LOTO procedures including, but not limited to shutting down the conveyor lines and did not lock out the energy sources. An employee suffered a multi-finger amputation.

The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. a) Dick's Sporting Goods Inc., 1314 Conklin Road, Conklin, NY - Throughout the facility. On or about April 22, 2023, maintenance technicians perform servicing of equipment with multiple sources of energy including but not limited to electrical, pneumatic, gravity and thermal. The employer did not certify that periodic inspections were performed or that the certification contained the information as required.
