Lake St. Amherst OH: The employer did not ensure that an aerial lift operator was tied off and secured to the platform before operating a JLG Telescopic Boom Lift (Aerial Lift). An employee utilizing aerial lift to performed steel erection work activities in this condition was exposed to falls of approximately 17 to the ground below.

1926.761(c)(3)(i) The nature of the hazards associated with work within a controlled decking zone; and 29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) Forest City Erectors, Inc. On or about April 25, 2023 at the address of 917 N. Lake St. Amherst OH 44001: The employer did not ensure employees were protected from hazards associated with falls while working atop a steel frame structure. Employee(s) performing steel erection work activities (securing/welding metal decking) were exposed to fall hazards of 16'- 8" or greater to the ground below.
