29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Southwest side of commercial structure; A 32-foot Werner brand model no. 5132 fiberglass extension ladder placed at ground level and leaned against the east-facing eave, was used by workers for roof access/egress and was not adjusted to allow for the side rails to extend at least 3 feet above the landing surface, was not equipped with a grasping device(s) and was not secured to prevent displacement.
