On or about April 26, 2023, Rob The Builder, LLC at jobsite located at 1010 Conant St. Maumee OH 43537, did not ensure that each affected employee used appropriate eye or face protection when performing lumber framing and sheeting work on a new build commercial structure while exposed to potential struck by hazards (flying particles/debris) while using DeWalt battery powered circular saw (Model: DCS570) and Paslode pneumatic nail guns.

29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about April 26, 2023, Rob The Builder, LLC at jobsite located at 1010 Conant St. Maumee OH 43537, did not ensure that employees were protected from falls while operating an MEC 45-AJ (S/N 10813931) mobile scaffold work platform (aerial lift). One employee was exposed to falls of approximately 12 feet and greater to the ground below in that the fall protection harness and lanyard was not utilized by the employee when using the aerial lift work platform to perform commercial carpentry work onsite.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.     On or about April 26, 2023, Rob The Builder, LLC at jobsite located at 1010 Conant St. Maumee OH 43537, did not ensure employees were protected from falls while performing commercial new build carpentry activities atop a low sloped commercial second level deck. Two employees were exposed to a 14 foot or greater fall to the ground below.

On or about April 26, 2023 Rob The Builder, LLC at jobsite located at 1010 Conant St. Maumee OH 43537, did not ensure that an employee operating a powered industrial vehicle (United Rentals Model G6-42A - ID #10253881 rough terrain forklift) was adequately trained and recertified on a regular basis.  Lack of PIV training can expose employees to potential struck-by hazards.
