29 CFR  1926.451(b)(1):Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports.   a) Jobsite located at 828 - 840 N. 2nd street, Philadelphia, PA.: The employer did not ensure that the working level of the supported tubular welded frame scaffold used by an employee to work on a concrete pillar was fully planked thus exposing the employee to slip and fall hazards of approximately 82 inches.   Condition noted on or about April 26, 2023.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) Jobsite located at 828 - 840 N. 2nd street, Philadelphia, PA.:  The employer did not train employees working from the first level of a supported tubular welded frame scaffold, who was exposed to an approximate 82 inches fall hazard, to recognize the hazards associated with working from scaffolds and to understand the procedures to control or minimize those hazards.
