29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals: a) Siska eyelet, grommet, rivet setting machine. On or about April 19, 2023 an employee used a foot pedal to operate a manual grommet machine. The foot pedal was not guarded to protect from falling objects or accidental steps.
