The employer failed to ensure the employee was protected from falling to that lower level by use of personal fall arrest system or guardrails.

P&R Renovations, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 1402998, citation number 1, item number 1a and was affirmed as a final order on (11/27/2019), with respect to a workplace located at 500 Barnes Avenue, Syracuse, NY 13207.

On or about April 26, 2023, an employee was using a portable ladder to access the roof. The employer failed to ensure the portable ladder's side rails extended 3 feet above the access point.
