Employees were conducting framing operations on a multi-family residential property without the use of a fall protection system, not using properly installed ladders while accessing and egressing the top plates of framed walls, lack of training, and a lack of Personal Protective Equipment (PPE).  This condition exposed employees to fall and struck-by hazards.  (b)Gold State Quality Construction 2, LLC at 1970 Russell Street, Spearfish, SD 57783: On or about and at times prior to April 26, 2023, the exposing employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person.
