29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: (a) On or about April 20, 2023, at 2800 7th Avenue North, Fargo, ND, 58102, the 5 Ton Harrington Hoist and Crane - Model TMH3-0560 - Serial 35739, in the laser cutting area of the Fabrication building, was not locked or tagged out while a contractor was using a scissor lift at elevated heights for pipe installation.
