29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a worksite located at 5975 Screaming Eagle Lane, Columbia, Missouri, an employee was exposed to head injury hazards.  The employer exposed the employee to these hazards by allowing the employee to work without appropriate head protection in an area where there was a possible danger of head injury from falling or flying objects.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a worksite located at 5975 Screaming Eagle Lane, Columbia, Missouri, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to work at heights greater than 6 feet without the use of fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a worksite located at 5975 Screaming Eagle Lane, Columbia, Missouri, employees were exposed to fall hazards.  The employer exposed the employees to fall hazards by allowing them to utilize portable extension ladders that did not extend at least 3 feet above the surface for which they were being used to gain access.
