29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  On or about April 17, 2023, in the converting room at converting machine #8, the employer did not insure hazardous energy control lockout procedures were utilized during machine maintenance functions that required employee to clear a jam. This exposed employees to ingoing nip points, crushing hazard and amputation hazard created by the energized and rotating pulley and belt assembly.  The Converting Technology, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1626230, citation number 1, item number 1 and was affirmed as a final order on March 08, 2023, with respect to a workplace located at 7737 North 81st Street in Milwaukee, WI.  The Converting Technology, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1579998, citation number 1, item number 1 and was affirmed as a final order on August 5, 2022, with respect to a workplace located at 7737 North 81st Street in Milwaukee, WI.
