29 CFR  1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about April 24, 2023, employees in the fabrication bay on the southside were utilizing an FMB Horizontal Bandsaw without the guards in the proper position to protect employees from the point of operation injuries and contact with the blade.

29 CFR  1910.243(a)(2)(iii)(a): All other hand-held powered tools, such as, but not limited to, platen sanders, grinders with wheels 2 inches in diameter or less, disc sanders with discs 2 inches in diameter or less, routers, planers, laminate trimmers, nibblers, shears, saber, scroll, and jig saws with blade shanks a nominal one-fourth of an inch wide or less, may be equipped with either a positive "on-off" control or other controls as described by paragraph (a)(2)(i) and (ii) of this section.  On or about April 24, 2023, employees in the fabrication bay an employee used a Dewalt 4 1/2-inch angle grinder without a guard while performing fabrication tasks.

29 CFR  1904.41(a)(2):29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in Appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The establishment employed 25 employees and is classified 332312-Fabricated Structural Metal Manufacturing during the calendar year 2022.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located, and identified so that they were readily accessible without subjecting the employees to injuries.   On or about April 24, 2023, materials blocked fire extinguishers in several locations and were not readily accessible to employees. a) On the northeast side of Plasma Bay.  b) On the northeast side of the welding/fitter bay.
