On or about April 26, 2023, employees used Bench grinders in several locations where the tongue exceeded the maximum allowable distance of 1/4 of an inch. a. On or about April 26, 2023, employees worked in an area of the fabrication shop where an electrical junction box is used to supply power to equipment located around the J-10 area was missing a knockout plug, exposing live inner conductors.
