Location: a) Supported scaffold on the front side of the building:  On or about April 26, 2023, two employees were exposed to a fall hazard of approximately 18 feet from the third tier of the scaffold to the front of the yard below. Employees used the third tier of the scaffold platform opening for hoisting sheet rocks delivery. The employer did not provide the employees with fall protection.

Location: a) On the third floor of the jobsite:   On or about April 26th, 2023, an employee was using an 8-foot A-frame stepladder in its closed position.
