29 CFR 1926.451(g)(1)(vii):For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Location: a) Supported scaffold on the front side of the building: On or about April 26, 2023, two employees were exposed to a fall hazard of approximately 18 feet from the third tier of the scaffold to the front of the yard below. Employees used the third tier of the scaffold platform opening for hoisting sheet rocks delivery. The employer did not provide the employees with fall protection.

29 CFR 1926.1053(b)(4):Ladders shall be used only for the purpose for which they were designed. Location: a) On the third floor of the jobsite: On or about April 26th, 2023, an employee was using an 8-foot A-frame stepladder in its closed position. The ladder was braced against the wall for support while climbing. The employee was not protected from falling by opening the spreader of the ladder.
