On a 10/12 steep pitched residential roof located at 1443 Walnut Street in Jacksonville Florida: On or about April 27, 2023, the employer exposed employees to fall hazards ranging from 6 to 16-foot, in that fall protection was not utilized by employees, while performing roofing work .
