29 CFR  1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a) On or about August 28, 2023, the employer did not ensure that employees implemented energy control application steps prior to performing setup activities on the National 316 H5 heading machine.  Hazardous energy was not isolated prior to employees engaging in servicing and maintenance activities such as machine setup.  This exposed employees to laceration and amputation hazards from unexpected movement of the machine.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) Production Area, Roller Department  On or about August 28, 2023 employees working on the Hartman Roller machine (R2) were exposed to amputation and crushing hazards when checking to ensure stock was forming properly by the moving die and when unjamming screws from the railing.  The employer did not adequately guard the machine to prevent employees from getting their hands / fingers caught in the moving parts of the machine.      b) Production Area, Header Department  On or about August 28, 2023 employees working on the National 316 Header machine  (H5) were exposed to ingoing nip point hazards when placing wire through the feed rollers of machine during setup and operation.  Employer did not adequately guard the feed rollers to prevent employees from getting their hands / fingers caught in the moving parts of the machine.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
