29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:   a.	On or about May 1, 2023, at 8300 W Beaver St. Jacksonville, FL 32220, the employer had not performed a job hazard assessment to evaluate the hazards present at the work site and what personal protective equipment was necessary to protect employees from those hazards.

On or about May 1, 2023, the employer exposed employees to hazardous chemicals such as but not limited to spray aerosol containing Perchloroethylene, deodorized kerosene, and carbon dioxide, and lubricant containing Phosphoric acid, isopropyl alcohol and ethoxylated, in that a written hazard communication program was not developed and implemented.

Production Area: On or about May 1, 2023, the employer exposed employees to hazardous chemicals that include but are not limited to spray aerosol containing Perchloroethylene, deodorized kerosene, and carbon dioxide and plastic container containing Phosphoric acid, isopropyl alcohol and ethoxylated, without maintaining safety data sheets on these chemicals as required by the standard.

On or about May 1, 2023, the employer exposed employees to hazardous chemicals such as but not limited to spray aerosol containing Perchloroethylene, deodorized kerosene, and carbon dioxide and plastic container containing Phosphoric acid, isopropyl alcohol and ethoxylated in that the employer did not provide information and training on the health and physical hazards of these chemicals.
