A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a-(2nd Site Visit to worksite at 8:50am)-1891 Greenwood Lake Turnpike, Hewitt NJ 07421: CSHO's observed an employee exit an unprotected excavation of approximately 48ft in length exposed to cave-in hazards in which a second means of egress had not been provided. Condition observed on or about 05/01/2023.
