W & O Supply Inc., located at 2677 Port Industrial Drive, Jacksonville FL: On or about May 2, 2023, and times prior thereto, the employer failed for calendar year 2022, to electronically submit information from the OSHA Form 300A or equivalent by 03/02/2023.
