29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   a) On or about 05/01/2023, throughout the establishment; Employer did not provide Lockout/Tagout (LOTO) training to authorized employees. Employees performed maintenance and servicing activities on machines such as, but not limited to: changing tools on planner mill, changing saw blade on table saw. Employees were not trained as LOTO authorized employees.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) On or about 05/01/2023, in the Machine Shop; employees were exposed to flying debris and rotating parts hazards when operating the Pratt Whitney lathe without guarding the chuck.   b) On or about 05/01/2023, in the Machine Shop; employees were exposed to flying debris and rotating parts hazards when operating the Niles lathe without guarding the chuck.   c) On or about 05/01/2023, in the Main Assembly Tool Crib area; employees were exposed to flying debris and rotating parts hazards when operating Pacemaker lathe without guarding the chuck. 29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about 05/01/2023, in the production area, employees were exposed to being struck by flying debris when using the Powermatic pedestal grinder with a tongue guard adjusted to more than 1/4 inch.
