The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) Top of the roof, South Wall: On or about May 2, 2023, the employer exposed employees to back injuries, in that the employees were only using body belts as their personal fall protection systems only while removing roofing material.
