29 CFR  1926.453(b)(1): 29 CFR 1926.453(b)(v):  Body belts were not worn when working from an aerial lift:   a)  At 1872 West Beach Boulevard, Building H Sea Oaks, Gulf Shores,  Alabama:  On May 2, 2023; the employer allowed his employees to use an aerial lift to gain access to a roof top and the crew was not tied off in the aerial lift exposing his employees to a fall hazard of up to 35 feet to the ground below.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At 1872 West Beach Boulevard, Building H Sea Oaks, Gulf Shores,  Alabama:  On May 2, 2023; the employer allowed his employees to perform roofing activities without first ensuring his crew was trained on the hazards associated with falls to include but not limited to setting up and inspecting fall protection exposing his employees to a fall hazard of 30 feet to the ground below.
