(p) Building - On or about May 2, 2023, and at times prior thereto, the employer did not have any documentation or proof of a rated load test of the crane (Crane: P-12103 - Hoist Serial# 163975). This exposed employees to struck-by and/or crushed by hazards in that the crane may be damaged internally, which may not be identified by a daily visible pre-use inspection. This exposed employees to struck-by and/or crushed by hazards in that the crane may be damaged internally, which may not be identified by a daily visible pre-use inspection. Without an official load test, the crane may fail resulting in a load drop.  (t) Building - On or about May 2, 2023, and at times prior thereto, the employer did not have any documentation or proof of a rated load test of the crane (Crane: P-13559 .B). This exposed employees to struck-by and/or crushed by hazards in that the crane may be damaged internally, which may not be identified by a daily visible pre-use inspection. Without an official load test, the crane may fail resulting in a load drop.  (u) Building - On or about May 2, 2023, and at times prior thereto, the employer did not have any documentation or proof of a rated load test of the crane (Crane: P-2101-B). This exposed employees to struck-by and/or crushed by hazards in that the crane may be damaged internally, which may not be identified by a daily visible pre-use inspection.
