29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a- 650 James Street, Lakewood NJ: Company workers were not protected from falling, up to an in excess of 15 feet to the concrete floor below, when walking and working from unsecured wooden planks that bridged between a commercial structure's third floor eye beams. Condition observed on or about 05/02/23. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) 650 James Street, Lakewood NJ: Company worksite workers were exposed to fall hazards, up to and in excess of 14 feet to the concrete flooring below, when working/walking from the top buck of a prefabricated, tubular welded framed scaffold that was not provided with guardrails. Observed on or about 05/02/23. A.M. Safety Construction, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1357281, citation number 1, item number 1 and was affirmed as a final order on 02/13/2019, with respect to a workplace located at 5369 Route 9 North, Howell NJ. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
