29 CFR 1926.416(a)(1):The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: (a) 6363 South Fiddler's Green Circle, Greenwood Village, CO 80111: On or about May 2, 2023, the employer E Light Electric Services, Inc., did not ensure its employees conducting a sub meter installation onto the main switchgear panel in an electrical room were protected from inadvertent contact with a live electrical source rated at 4000 amp/ 480v-660v. An employee sustained electrical burns to the face, neck and arms from an arc flash.

29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: (a) 6363 South Fiddler's Green Circle, Greenwood Village, CO 80111. On or about May 2, 2023, the employer, E light Electric Services, Inc., did not ensure its employees conducting a sub meter installation onto the main switchgear panel of an electrical room were protected from shock and electrical burns, in that the panel was energized and not locked or tagged out. An employee sustained electrical burns to the face, neck and arms from an arc blast. After the incident, additional employees were exposed to potential injury in that, the panel was de-energized; however, no locks or tags were placed on the controls to prevent reenergization.
