29 CFR  1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards.  a) 6832 Ridge Ave. Philadelphia, PA 19128, north bound lane - On or about May 3, 2023, the employer did not ensure the construction area was posted with legible traffic signs at points of hazards.  Employees were re-directing traffic for an active excavation site along the street and sidewalk without traffic signs thereby exposing employees to struck-by hazards.  b) 6832 Ridge Ave. Philadelphia, PA 19128, south bound lane - On or about May 3, 2023, the employer did not ensure the construction area was posted with legible traffic signs at points of hazards.  Employees were re-directing traffic for an active excavation site along the street and sidewalk without traffic signs thereby exposing employees to struck-by hazards.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) Jobsite located at 6832 Ridge Ave. Philadelphia, PA 19128 - On or about May 3, 2023, the employer did not ensure the excavation measuring approximately 15 feet in depth was protected from cave-ins by an adequate protective system. Gaps of approximately 2 feet existed between the shoring panels exposing an employee working within the gap to struck-by hazards.  Broderick Industries LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1411251, citation number 1, item number 1 and was affirmed as a final order on 1/14/2020, with respect to a workplace located at 2508 Grays Ferry Avenue, Philadelphia, PA 19146.
