29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a) On or about May 3, 2023, employees were exposed to fall hazards of approximately 9-feet and greater when they used an aluminum extension ladder that was not extended three feet above the roof landing.
