The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   On or about May 3, 2023, at 891 Co Rd U in Wrightstown, Wisconsin, the employer did not provide required fall protection safety training for employees exposed to fall hazards greater than six feet while installing water proofing materials on second floor patios.  Fall protection not required on any level surface.

29 CFR  1926.1060(a)(1)(iii):  Training Requirements   The employer shall provide a training program for each employee using ladders and stairways, as necessary.  The program shall enable each employee to recognize hazards related to ladders and stairways and shall train each employee in the procedures to be followed to minimize these hazards.  The employer shall ensure that each employee has been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders.  On or about May 3, 2023, at 891 Co Rd U in Wrightstown, Wisconsin, the employer did not provide required ladder safety training for an employee exposed to a fall hazard while using an extension ladder installing water proofing materials on second floor patios.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about May 3, 2023, at 891 Co Rd U in Wrightstown, Wisconsin, an employee was exposed to s fall hazard while working on second floor patios 10.9 feet above surface level without using fall protection equipment. 29 CFR  1926.1053(b)(7):  Ladders shall not be used on slippery surfaces unless secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery.   On or about May 3, 2023, at 891 Co Rd U in Wrightstown, Wisconsin, an employee was exposed to a fall hazard while working from an extension ladder 7.5 feet above surface level without the the ladder level and the base protected from slipping.  Ladder base on one flipped up shoe and one fly section corner.

On or about May 3, 2023, at 891 Co Rd U in Wrightstown, Wisconsin, an employee was exposed to a potential fall hazard while working from an extension ladder 7.5 feet above surface level positioned in a doorway without securing the ladder or using a barricade.
