On or about May 02, 2023, and at times prior, employees working on a two-story residential townhome roof with a slope greater than 4:12 at the landing surface were exposed to falls over 6 feet without the use of fall protection.

On or about May 02, 2023, and at times prior, employees accessing the upper landing surface of a two-story townhome roof to perform roofing activities used a ladder that did was unsecured at the top, exposing employees to a fall.
