Violation occurred on or about May 4, 2023.

Location: 128 Monroe St. Boonton, NJ 07005 a) Employer did not ensure that the exact location of underground installations was determined by safe and acceptable means during excavation operations. A utility gas pipe was struck by a compact excavator backhoe bucket operated by the employer. Violation was observed on May 4, 2023.
