29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: 720 Southampton Road, Westfield, MA: On May 4, 2023, and at times prior, the employer allowed employees to operate a Mitsubishi fork truck , Model FGC25N, serial # AF82F30495 that was equipped with rotating fork attachment that was not factory installed and not requested that the truck have an updated data plate be installed showing the new load capacities with the attachment exposing employees to tip and roll over hazards.
