Forklift, 2018 Nissan MCP1F2A25LV, Serial #CP1F2-WZ0648 and forklift, 2018 Nissan MCP1F2A20LV, Serial #CP1F2-9P9388 with damaged, unreadable safety/warning stickers actively operated at the facility had Danger, Warning and Caution labels which were missing, damaged, or illegible.

29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval: (a) On or before 5/12/2023 the employer was allowing the operation of a Forklift, 2017 Mitsubishi FGC15N, Serial #AF81F00703 while missing one of the tie downs on propane tank strap. The forklift originally was designed with two tie downs and two straps to secure the propane fuel tank on an LPG model.
