29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a) Electralloy G. O. Carlson, Inc. 175 Main St. Oil City, Pa 16301- On or about 05/04/2023 and at times prior thereto, employees were exposed to 14 foot fall hazards when accessing the crucible, hopper, and framework for assembly and disassembly of the ESR #1 and ESR #2 re-melting furnaces. Employees were not using any fall protection systems during assembly and disassembly of the ESR's.

29 CFR 1910.28(b)(2)(ii): The employer did not ensure that when any portion of a guardrail system, gate, or chains is removed, and an employee must lean through or over the edge of the access opening to facilitate hoisting, the employee is protected from falling by a personal fall arrest system. a) Electralloy G. O. Carlson, Inc. 175 Main St. Oil City, Pa 16301- On or about 05/04/2023 and at times prior thereto, employees were exposed to 14 foot fall hazards when removing the guardrails on the ESR#1 and ESR#2 Re-melting Furnaces. Employees remove the guardrail system to facilitate the assembly and disassembly of the ESR's.
