29 CFR  1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:  a) Facility - On or about June 8, 2023, an employee, Polisher, was exposed to continuous noise at 187.3 % of the permissible exposure level of 90 dBA or an eight-hour time-weighted average of 94.5 dBA. This exposure was observed over a 669 minute sampling period. The employer did not implement feasible administrative and/or engineering controls.   b) Facility - On or about June 8, 2023, an employee, Polisher, was exposed to continuous noise at 337.4 % of the permissible exposure level of 90 dBA or an eight-hour time-weighted average of 98.7 dBA. This exposure was observed over a 690 minute sampling period. The employer did not implement feasible administrative and/or engineering controls.     No abatement certification or documentation required.

29 CFR  1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:  a) Facility (Next to Annealing Machinery) - On or about May 4, 2023, the flammable chemical Martol was dispensed by gravity from a container that did not have a self-closing valve.  No abatement certification or documentation required.
