29 CFR  1926.25(a) :During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  On or about May 4, 2023, the employer failed to ensure the area around the bottom of the ladder, used to access and egress the roof working area, was kept cleare of debris from roof  tear off.  The bottom of the ladder was scattered with roofing debris , creating a slip and trip hazard.  No abatement documentation is required for this item.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 4, 2023,  the employer failed to ensure the ladder that the employees  were utilizing to access and egress the roof of the residential structure was positioned to extend 3 feet above the landing edge.  The employees were utilizing the ladder while performing residential roofing activities.  No abatement documentation is required for this item.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:  On or about May 4, 2023,  the employer failed to ensure that the employees engaged in residential roofing activities were properly protected from falls in excess of 10 feet.  The employees engaged in residential roofing activities were not utilizing any fall protection.  J & N Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1395486, citation number 1, item number 2 and was affirmed as a final order on June 28, 2019, with respect to a workplace located at 14142 Woodward Dr., Orland Park, IL.  No abatement documentation is required for this item.

29 CFR  1926.503(b)(1):The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  On or about May 4, 2023 the employer failed to provide written documentation showing that the the employees have been trained in the recognition and mitigation of fall hazards.  The employees were engaged in residential roofing activities at a height in excess of 10 feet with using fall protection.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
