29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section: On or about May 4, 2023, and times prior thereto, at this Falls City location, the employer failed to develop and implement sufficient lock-out/tag-out procedures to provide enough direction so that an employee could safely perform maintenance work inside of a cement hopper on a 2022 Mack 40X-57R DY Cement Pump Truck (Unit P1, License plate PZK3319) thus exposing the employee to caught in hazard.
