29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. At a jobsite located at 2601 N Arkansas Ave, Wichita, KS 67204, employees conducting commercial roofing activities were exposed to serious fall hazards of approximately 16 feet from the ceiling joists to the ground below in that fall protection was not implemented nor a monitoring system.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at 2601 N Arkansas Ave, Wichita, KS 67204 employees engaged in residential roofing activities were exposed to fall hazards when accessing an extension ladder: 1) An aluminum extension ladder utilized at the residential building was 16 feet from the eave of the residence to the ground below. Employees accessed and exited the roof utilizing the portable ladder and it did not extend at least 3 feet above the upper landing and was not secured to the structure.
