29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person(s)  a) On May 2, 2023, at 1301 Administration Avenue, Ceres Hall, NDSU campus:    (1) Sub-contractor employees were exposed to a Fall hazard when the working surface of the scaffold was at a height of over 10 feet and was not fully planked.   (2) Sub-contractor employees were exposed to a fall hazard while working on a scaffold at heights greater than 10 feet without guard rails or personal fall arrest systems in place.   (3) Sub-contractor employees were working on scaffolding and did not provide protection for falling objects.   (4) Sub-contractor employees were on a walking/working roof surface at a height greater than 6 feet and were exposed to a fall hazard where no protection was being used.
