29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  (a) On May 2, 2023, at 1301 Administration Avenue, Ceres Hall, NDSU Campus, Fargo, ND 58105, employees were exposed to a Fall hazard when the working surface of the scaffold was at a height over 10 feet and was not fully planked.

(a) On May 2, 2023, at 1301 Administration Avenue, Ceres Hall, NDSU Campus, Fargo, ND 58105, employees were exposed to fall hazards while working from a scaffold at heights greater than 10 feet without guard rails or personal fall arrest systems in place.

29 CFR  1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy, or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  (a) On May 2, 2023, at 1301 Administration Avenue, Ceres Hall, NDSU Campus, Fargo, ND 58105, employees working on and/or below scaffolding were not protected from falling objects.

(a) On May 2, 2023, at 1301 Administration Avenue, Ceres Hall, NDSU Campus, Fargo, ND 58105, employees who were installing sheet metal roofing at heights greater than 6 feet were exposed to fall hazards with no protective systems in place.
