PTV Contracting, LLC, 4203 W Ridge Rd, ERIE, PA 16506, on or about 5/5/23 and times prior thereto: site workers were exposed to impalement hazards due to unprotected protruding rebar. Rebar was located between masonry walls. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
