29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: (a) 3rd floor of Phase 3 building: On or about May 2, 2022, employees walking and working to install framing materials were exposed to fall hazards, in that ,an open floor trash chute measuring approximately 36-inches by 72-inches and 21-feet deep was covered but was not secured to prevent movement.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) 3rd floor of Phase 3 building: On or about May 2, 2022, the employer did not ensure employees engaged in framing and were regularly exposed to fall hazards up to approximately 21 feet onto earthen ground were effectively trained to recognize the fall hazards encountered in their work.
