29 CFR  1910.151(c):Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [63 FR 33450, June 18, 1998]  On or about May 2, 2023, the employer failed to ensure that a suitable eyewash station was provided in the shop area exposing employees to injurious eye hazards. There was not other eye wash station nearby and the eye wash bottles were expired.

29 CFR  1910.179(j)(2)(iii):Hooks with deformation or cracks. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10�� twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section.  On or about May 2, 2023, the employer did not conducted monthly inspection on the crane hooks (crane S/N-30510M002) and the employees were lifting equipment included but not limited to BOP's.

29 CFR  1910.178(q)(7):Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.
